How to Apply for a Certificate

1. Make sure to first read the rules of the program.

2. Print and fill out a checklist and application sheet.

Both the checklist and application sheet can be viewed and printed using Adobe Acrobat Reader which is available FREE online.  Just click on the logo below.



3. Send to:
